Global Consensus on Management of Pregnancy in IBD
We now have international recommendations on management of Inflammatory Bowel Disease from pre-pregnancy, through pregnancy, to delivery and post-partum. Topics include fertility, IBD medications, obstetrical considerations (e.g. low dose Aspirin for risk of preeclampsia), and infant health and vaccinations (e.g. Rotavirus).
Some key recommendations include:
- continuing biologics to manage IBD throughout pregnancy without any need to time or delay dosing
- providing low dose ASA in pregnant IBD to reduce the risk of preterm preeclampsia
- discontinuing small molecules (JAK inhibitors and S1P modulators) before pregnancy unless there are no alternatives to maintain maternal health
- giving live Rotavirus vaccine to infants even if exposed to biologics
